Home Health Registered Nurse

**Overview** As a Home Health Registered Nurse, you’ll bring skilled, compassionate care directly to patients in their homes. You’ll assess health needs, provide treatment, coordinate services, and help patients and caregivers understand the care plan. Your work supports safer recovery and greater continuity of care in a familiar setting. **Duties** * Your impact: By identifying changes in a patient’s condition and coordinating timely follow-up, you help patients and caregivers make informed decisions and maintain continuity of care. * Complete patient assessments, monitor vital signs, and document findings in electronic medical records (EMR/EHR). * Develop and update care plans; administer medications, injections, and intravenous (IV) infusions as ordered. * Provide home care such as catheter and ostomy care, tube feeding, wound care, and infection control using aseptic technique. * Coordinate case management, intake, discharge planning, and referrals with patients, caregivers, and the care team. * Teach health coaching and safety practices, including equipment use and basic life support (BLS) response when needed. **Qualifications** * Required: Current registered nurse license and ability to provide patient-centered nursing care in home settings. * Required: Patient assessment, medication administration, medical documentation, HIPAA (Health Insurance Portability and Accountability Act) awareness, and familiarity with EMR/EHR systems. * Preferred: Home care, senior care, geriatrics, hospice and palliative medicine, long-term care, assisted living, memory care, or nursing home experience. * Preferred: Experience with IV insertion, catheterization, phlebotomy, venipuncture, triage, acute pain management, dialysis, or developmental disabilities. * Additional relevant experience may include Epic, Cerner, eClinicalWorks, medical-surgical, acute or critical care, emergency medicine, pediatrics, behavioral health, and ICD-10 coding. Pay: From $60.00 per hour Expected hours: No less than 20.0 per week Work Location: On the road
