Case Manager

Responsibilities * Outreach to and engage eligible clients * Establishing trusting relationships with clients and their families while providing general support and encouragement. * Provide ongoing follow\-up, basic motivational interviewing and goal setting * Provide referrals for services to community agencies as appropriate * Receive daily referrals of members at risk for housing from (Magellan/AIP unit) * For members who agree to be part of the CBCM program: meet prior to discharge and schedule and meet with member within 5 days of discharge * Document encounters, assessments and progress notes, and plan interventions; track processes and outcomes, participate in activities to design and implement standard policies and protocols; prepare case reports and assist with required reports. * Assigned specific days and hours to support members at the Conference drop\-in center. Qualifications 1\. Ability to work with low income individuals and families utilizing knowledge of community resources. 2\. Knowledgeable about community resources appropriate to needs of patients/families. 3\. Be SOAR certified or be willing to become SOAR certified. 4\. Social service experience. 5\. CRS or CPS certified individuals encouraged to apply. Staff requirements: * Attend all staff meetings, training, retreats, and Conference annual meetings. * Work as a team with all Conference staff. * PA Driver’s license and access to own transportation. * All other tasks as requested by supervisor.
