Physical Therapist

About Us We are seeking a compassionate, motivated, and skilled Physical Therapist to join our growing home health team. Our mission is to provide high-quality, patient-centered care that helps individuals regain mobility, improve independence, and achieve the best possible quality of life in the comfort of their own homes. Job Responsibilities * Perform comprehensive physical therapy evaluations and develop individualized treatment plans. * Provide therapeutic exercises, gait training, balance training, transfer training, and functional mobility interventions. * Help patients recover from surgery, illness, injury, or other conditions affecting mobility and daily function. * Monitor patient progress and adjust treatment plans as needed. * Educate patients and caregivers on home exercise programs, safety precautions, and fall prevention. * Document evaluations, treatment sessions, progress notes, and discharge summaries accurately and promptly. * Collaborate with physicians, nurses, occupational therapists, and other members of the interdisciplinary care team. * Maintain compliance with applicable state and federal regulations, agency policies, and professional standards. * Travel to patients' homes within the assigned service area. Qualifications * Current, unrestricted Physical Therapist license. * Doctor of Physical Therapy (DPT) or other qualifying degree from an accredited physical therapy program. * Strong clinical assessment, communication, and patient-care skills. * Ability to work independently while collaborating effectively with a multidisciplinary team. * Reliable transportation and a valid driver's license, as applicable to the service area. * Home health experience preferred, but motivated new graduates may be considered. * Familiarity with electronic medical records and home health documentation requirements preferred.
