### **A rehabilitation and skilled nursing facility in Brooklyn is seeking a Social Worker (SW) to join its care team.** ### **Duties:** * Maintain and/or improve residents’ ability to manage physical, mental, and psychosocial needs * Identify individual social and emotional needs * Develop and maintain individualized care plans * Participate in admission planning, community re\-entry, and transfers * Attend interdisciplinary care plan meetings to support residents’ emotional and social well\-being ### **Requirements:** * Bachelor’s Degree in Human Services, Psychology, Social Work, or related field * MSW required * Long\-term care or subacute experience required * Bilingual Chinese\-speaking candidates preferred ### **About the Facility:** This well\-established rehabilitation and skilled nursing center provides comprehensive care in a supportive and resident\-focused environment.
