**Faith & Hope Hospice** prides itself on hiring Registered Nurses that can provide the most compassion and drive and are currently hiring staff for our **Registered Nurse** team. We have been steadily growing since 2007 and our goal is to challenge and retain the members of our organization. **\*Hospice experience required\*** **Registered Nurse Job Description:** * Regularly assesses and reassesses the nursing needs of the Hospice patient. * Provides Hospice nursing services, treatments, and preventive procedures. * A registered nurse should observe signs and symptoms and report any unexpected changes in the patient’s physical or emotional condition to the physician and IDT members. * Develops and re-evaluates the patient/family care plan in conjunction with IDT to meet needs and maintain continuity of care. * Performs specific registered nurse procedures as needed (e.g., treatments, management of symptoms) following doctor’s orders. * Maintains records as required by Hospice. * Coordinates the implementation of the plan of care for patients residing in SNF, NF, ICF or MR.
