Case Manager: Full Time

**Cascade Medical** , in beautiful downtown Leavenworth in the foothills of the Cascade Mountains, is looking for a full time **Case Manager** . **Job Summary:** The **Case Manager** coordinates patient care and discharge planning across inpatient, observation, and Swing Bed services. Working with patients, families, providers, nursing, rehabilitation, social work, registration, and community partners, this role identifies barriers early, supports appropriate admissions and transitions, and promotes safe, timely, and well-coordinated care. The Case Manager maintains clear documentation and helps patients receive the services, equipment, follow-up, and information needed after discharge. **Work Schedule:** Schedule is anticipated to be 40 hours per week Monday-Friday along with occasional weekend shifts.
