**Company Description** FERI\-TB is a Virginia\-based 501(c)(3\) nonprofit, founded in 2025, dedicated to ending tuberculosis across Virginia and Maryland. The organization focuses on removing structural barriers such as housing, employment, transportation, and language that impede treatment completion. FERI\-TB advances health equity, dignity, and recovery for people affected by TB by integrating social support with clinical care. The organization offers a mission\-driven work environment for individuals committed to public health and social justice. **Role Description** The Patient Navigator is a full\-time, on\-site role based in Baltimore, MD. Day\-to\-day responsibilities include conducting needs assessments, coordinating appointments, accompanying or arranging transport to clinical visits, and providing clear, culturally responsive information about treatment and follow\-up. The Patient Navigator works closely with healthcare providers, social service agencies, and community organizations to reduce barriers to care and promote treatment adherence. The role also involves maintaining accurate documentation, tracking patient progress, and reporting on outcomes to support program improvement. **Qualifications** * Demonstrated skills in patient advocacy, care coordination, and navigation of healthcare and social service systems. * Strong communication, interpersonal, and cultural humility skills, with the ability to build trust across diverse communities. * Organizational and time\-management skills, including accurate documentation, follow\-up, and use of basic office or electronic record tools. * Ability to problem\-solve around barriers such as housing, transportation, employment, and language access, and to connect individuals to appropriate resources. * Commitment to health equity, trauma\-informed practice, and nonjudgmental support for people affected by TB and related social challenges. * Experience working in community health, public health, social work, or related fields is strongly preferred. * Familiarity with Baltimore\-area community resources and willingness to travel locally as part of the role. * High school diploma or equivalent required; an associate or bachelor’s degree in a health, human services, or social science field is a plus. * Bilingual or multilingual skills, especially in languages commonly spoken in Baltimore’s communities, are highly beneficial.
