Director of Social Work (DSW)

A respected long\-term care organization is seeking an experienced **Social Services Director** to lead the Social Services Department at one of our skilled nursing facilities. This leadership role is responsible for overseeing department operations, ensuring regulatory compliance, and providing high\-quality psychosocial support to residents and their families. ### **Key Responsibilities** * Provide leadership, direction, and oversight for the Social Services Department. * Conduct resident assessments and develop individualized care plans. * Assist residents and families with adjustment, support, and transition throughout their stay. * Supervise, mentor, and support Social Services staff. * Coordinate discharge planning beginning at the time of admission. * Collaborate with the interdisciplinary care team to ensure quality resident outcomes. * Participate in community outreach and develop relationships with external agencies as needed. * Ensure compliance with all applicable federal, state, and facility regulations. * Exercise sound judgment, initiative, and problem\-solving skills in a fast\-paced healthcare environment. ### **Qualifications** * Minimum of 2 years of management experience in a long\-term care or skilled nursing setting. * Current LSW required; MSW preferred. * Strong knowledge of state and federal regulations governing long\-term care. * Excellent leadership, communication, and interpersonal skills. * Ability to manage multiple priorities while maintaining a resident\-centered approach.
