HIV Community Liason

LaSante Health Center is seeking a dedicated HIV Community Liaison to support individuals living with HIV and enhance community engagement. The ideal candidate will have strong communication skills, knowledge of HIV\-related issues, and a passion for advocacy. Responsibilities: * Support individuals living with HIV and connect them to care. * Conduct outreach and educational initiatives. * Build relationships with community organizations. * Advocate for patients and address barriers to care. * Provide HIV\-related education and resources. * Collaborate with healthcare teams to support patients. * Maintain records and complete assigned tasks. Qualifications: * Experience in community outreach or healthcare. * Knowledge of HIV prevention and support services. * Strong interpersonal and organizational skills. * Bilingual in Haitian Creole (preferred).
