In this role, you will provide outpatient physical therapy services directly at patients' locations, ensuring personalized care and effective treatment plans. This position offers a dynamic work environment where mobility and patient engagement are central to delivering exceptional care. **Responsibilities** * Deliver outpatient physical therapy services at patients' homes or other designated locations, tailoring treatments to individual needs. * Conduct thorough assessments and develop personalized treatment plans based on patient evaluations. * Document all therapy sessions accurately and efficiently, maintaining compliance with healthcare standards and organizational policies. * Engage with patients professionally, demonstrating a confident demeanor when discussing assessment findings and treatment progress. * Foster positive social interactions with patients to enhance their comfort and satisfaction with care. * Participate in marketing initiatives as approved, promoting the benefits of outpatient physical therapy services within the community. * Collaborate with healthcare team members to ensure comprehensive patient care and optimal outcomes. **Experience** * Prior outpatient or home health physical therapy experience is required, or a desire for this setting, and be able to demonstrate proficiency in assessment, treatment planning, and documentation. * Strong interpersonal skills are essential; candidates should be highly likeable and adept at social interactions. * Ability to confidently discuss assessment findings with patients and motivate them throughout their recovery process.
