**LPN Nurse Case Manager** **Abraxas Home Care, LLC** Abraxas Home Care, LLC is seeking a compassionate, organized **Licensed Practical Nurse (LPN) Nurse Case Manager** to support individuals with intellectual and developmental disabilities in our residential and community-based programs. This position combines hands-on nursing, healthcare coordination, and collaboration with individuals, families, healthcare providers, and Direct Support Professionals (DSPs). The ideal candidate communicates clearly, follows through on healthcare needs, and is committed to respectful, person-centered care. **What You’ll Do** * Provide nursing care and monitor individuals’ health within the LPN scope of practice and under appropriate clinical direction. * Recognize and report changes in condition and coordinate timely follow-up. * Coordinate medical appointments, referrals, laboratory testing, and preventive healthcare. * Communicate with physicians, pharmacies, specialists, legal guardians, and the support team. * Review medical orders, appointment summaries, and hospital discharge instructions to support continuity of care. * Review medication records, assist with medication reconciliation, coordinate refills, and resolve discrepancies with providers and pharmacies. * Maintain accurate nursing notes, health records, and care coordination documentation. * Participate in individualized support planning and team meetings. * Educate DSPs on approved health support instructions, infection prevention, and recognizing changes in condition. * Complete assigned health and medication audits and help address identified concerns. * Travel between service locations and attend medical appointments as needed. **Required Qualifications** * Graduate of an approved practical nursing program. * Current authorization to practice as an LPN in Virginia through a Virginia license or applicable multistate privilege. * Current CPR/BLS certification. * Strong communication, organization, documentation, and follow-up skills. * Ability to work collaboratively while managing an assigned caseload. * Comfort using electronic health records and basic office software. * Valid driver’s license and reliable transportation for required local travel. * Ability to complete required background checks and employment screenings. **Preferred Qualifications** * Two or more years of nursing experience. * Experience supporting individuals with intellectual or developmental disabilities. * Background in residential services, community nursing, home care, or behavioral health. * Experience with healthcare coordination, medication management, or Virginia DBHDS-licensed programs. **Who We’re Looking For** You are dependable, approachable, and attentive to detail.
