**Overview** Join our dynamic healthcare team as a Home Health Case Manager RN Part time available 3 days a week Flexible schedule, Flexible with accommodation. \*\*MUST come to the office for work\*\* Hybrid (visiting patients and working in the office is possible, \*\* will be separate payment and hours) **Responsibilities** * 485 * QA (nurses' chartings/visit notes) * create orders * POC (create/edit) * Organize charts **Experience** * Hospice / Home Health Case Manager or similar * SNF/Rehab intake coordinator, RN supervisor (create charting/intake assessment) \*\* Must hold current RN license Job Types: Full-time, Part-time Benefits: * Flexible schedule * Paid time off Work Location: In person Job Types: Full-time, Part-time Pay: $35.00 - $65.00 per hour Work Location: In person
