Come join an amazing team! **Requirements for Licensed Social Worker:** * Must possess a B.A in Social Work, LSW or equivalent degree AND a current social work license. * LSW, LCSW, or LICSW required. * 1+ years skilled nursing facility experience preferred. **Job Duties of Licensed Social Worker:** * Compiles social history data. * Participates in assessments, development and evolution of resident's care plan. * Responsible for planning and leadership interdisciplinary care planning weekly meetings. * Documents timely progress notes according to state and federal regulations. * Coordinates the discharge planning for residents. * Able to interpret and implement established social service policies, procedures and regulations. * Coordinates services of community agencies for continued delivery of care upon discharge.
