Full-Time Case Manager

**We are seeking a *Case Manager* to provide essential services within a comprehensive case management model.** **Key Responsibilities Include** * Managing client intake and assessment to identify needs and develop personalized service plans. * Collaborating with clients to set and update goals, and facilitating access to necessary services and benefits. * Providing supportive counseling to empower clients in making informed decisions. * Documenting case activities, collecting data, and participating in quality assurance efforts. **Qualifications** * Associates Degree or 60 college credits in Human Services, or High School Diploma/GED with relevant shelter experience. * Strong verbal and written communication skills, including proficiency in basic computer skills. * Ability to communicate professionally in both English and Spanish. **Ideal candidates are compassionate, organized, and committed to supporting individuals experiencing homelessness, domestic violence survivors, youth aging out of foster care, veterans, seniors, or children with developmental delays.**
