We are committed to accessible, low-barrier, and culturally responsive care for individuals, families, and communities. Our team values collaboration, compassion, clinical excellence, and continuity of care. We are especially committed to supporting patients with trauma histories and complex needs who benefit from coordinated community services. About the Role We are seeking an experienced Case Manager to provide direct patient care, help patients access treatment, connect them with community resources, and address practical barriers to care. This role manages a caseload, coordinates with the care team, follows through on referrals, and documents services accurately. The Case Manager is a direct service provider, with most work time devoted to direct patient case management services. The remaining time includes documentation, care coordination, referrals, and follow-up. We value dependable attendance, sound judgment, and respectful communication with patients and colleagues. Patient Support and Care Coordination · Identify patient needs and barriers and develop and update case management goals with the patient and care team. · Connect patients with housing, food assistance, transportation, insurance and benefit programs, primary care, and other community resources. · Help patients complete applications, navigate referrals, and understand next steps. Coordinate appointments, transportation, and language assistance, and follow up to confirm access to services and address remaining barriers. · Coordinate with psychiatric providers, therapists, hospitals, primary care offices, authorized support people, and community agencies to support the patient's care plan. · Assist with hospital discharge follow-up, transitions between providers, warm handoffs, and referrals. Conduct outreach after missed appointments or gaps in care. · Coordinate with schools, child welfare, probation, and other referral partners when appropriate. Documentation and Productivity · Complete timely, objective notes in the electronic health record describing the identified need, service provided, patient response, progress or barriers, and follow-up plan. Document unsuccessful outreach and declined services accurately. · Record actual service time and complete billing information according to payer requirements and Elevate procedures. Distinguish qualifying billable services from administrative and other nonbillable work. · Keep referrals, releases of information, chart updates, and follow-up tasks current. Communicate unresolved items to the supervisor and relevant care team members. · Meet established caseload, service, documentation, and productivity expectations while maintaining accuracy and providing services appropriate to each patient's needs. Required Qualifications · Prior experience in behavioral health, case management, care coordination, nursing, or a related healthcare or human services role involving direct patient support and documentation. · Bachelor's degree in a relevant field, or an associate degree with at least two years of case management experience. · Relevant education or coursework in social work, psychology, nursing, behavioral health, human services, or a related field. · Experience supporting individuals with complex behavioral health and social needs. · Ability to write clear, accurate, professional notes and follow documentation requirements. · Ability to meet applicable, job-related background screening requirements, consistent with state and federal law. Preferred Qualifications · Experience with Colorado Medicaid, community resources, and benefit programs. · Working knowledge of common mental health and substance use conditions and how they may affect daily functioning, engagement, and access to care. · Understanding of time-based service documentation. · Understanding of health insurance benefits, referral processes, and available assistance programs, including how to help patients navigate access to services. · Proficiency with electronic health records, email, scheduling systems, and routine computer tasks. · Knowledge of Colorado community resources and available programs in the practice's service area, with the ability to identify additional resources based on patient needs. Professional Expectations · Maintain reliable attendance and punctuality, follow through on assigned tasks, and communicate promptly when barriers may affect patient follow-up. · Organize competing priorities, respond to time-sensitive needs, and work independently while seeking supervision when needed. · Use respectful communication, maintain professional boundaries, and support patient choice. · Promptly communicate safety concerns, changes in symptoms, and medication questions to the appropriate clinician or supervisor. Follow established escalation procedures. · Protect confidential information and follow HIPAA, other applicable privacy requirements, and Elevate policies. Participate in supervision, training, and documentation reviews. · Provide case management services under the clinical supervision and oversight of Elevate's designated qualified licensed provider, consistent with applicable Medicaid and other payer requirements and Elevate policies. · Perform other related duties consistent with the role, training, and program requirements, as assigned.
