We’re growing and looking for an experienced **Registered Nurse (RN)** to join our **Hospice** **Care** team! **What You’ll Do** As a **Hospice RN**, you will plan, organize, and direct **hospice care** while educating patients and families. You’ll serve as a case manager and ensure quality, compassionate care in the home setting. **Key Responsibilities:** * Complete initial and ongoing patient and family assessments * Develop and revise individualized hospice care plans * Provide professional nursing care using the nursing process * Administer medications and treatments as prescribed * Educate and counsel patients and families on hospice care * Coordinate care for your assigned caseload **What We’re Looking For** * Active CA RN License * Minimum 2 years of nursing experience (hospice or home health experience preferred) * Strong clinical, communication, and problem-solving skills * Current CPR certification * Valid driver’s license, insured vehicle in good condition * Ability to work independently and manage priorities **Why Join Sequoia?** Sequoia Hospice is part of the Cornerstone Group, a network of 75 agencies nationwide.
