This position offers an excellent opportunity for RNs seeking supplemental income and flexible work arrangements. **Duties** * Conduct comprehensive in-home patient assessments to evaluate health status and care needs * Gather detailed information on clients’ physical, emotional, and environmental conditions * Collaborate with clients and their families to develop personalized care plans aligned with their needs and preferences * Document assessment findings accurately and maintain detailed records in compliance with agency policies * Provide guidance and education to clients and caregivers regarding health management and safety practices * Coordinate with other healthcare professionals as necessary to ensure continuity of care * Maintain confidentiality of all client information in accordance with privacy regulations **Qualifications** * Valid Registered Nurse (RN) license in the state of practice * Proven experience in patient assessment, preferably within a home care or community setting * Strong interpersonal and communication skills, with the ability to connect empathetically with clients * Ability to work independently with minimal supervision * Flexibility to accommodate variable scheduling needs, including evenings or weekends if required * Knowledge of healthcare regulations, safety protocols, and documentation standards * Reliable transportation and the ability to travel to clients’ homes as needed This position is ideal for registered nurses seeking a flexible role that allows them to make a meaningful difference in clients' lives while maintaining a manageable schedule.
