What You'll Do **As a Travel Home Health PT, you will:** • Complete physical therapy evaluations in patients' homes • Develop individualized plans of care • Provide therapeutic exercise, gait, balance, transfer, and mobility training • Help patients improve strength, endurance, mobility, and independence • Assess home safety and identify fall risks • Develop and reinforce home exercise programs • Educate patients, families, and caregivers • Monitor patient progress and adjust treatment plans as needed • Complete reassessments and discharge planning • Collaborate with nurses, physicians, therapists, and other members of the interdisciplinary team • Complete accurate and timely documentation using WellSky What We're Looking For • Active Virginia Physical Therapist License Required • Graduate of an accredited Physical Therapy program • Valid Driver's License and reliable transportation • Strong clinical and communication skills • Ability to independently manage home health visits • Strong time\-management and organizational skills • Home Health Experience Preferred, but Not Required • WellSky Experience is a Plus Why Take This Assignment?
