Case Manager

**Job description** **Case Management services plays a crucial role in supporting individuals with mental health challenges by coordinating care, advocating for services, and ensuring clients receive the support they need to maintain stability and improve their quality of life. Experience or interest in working with adults with a mental health diagnosis desired. Multi\-disciplinary team of psychiatrists, therapists, nurses and social workers.
