Physical Therapist Assistant - Home Health

Description: **About the Role:** Sierra Home Health is seeking a Physical Therapist Assistant (PTA) to join our compassionate and collaborative home health team. As a valued member of our tight-knit staff, you'll enjoy flexible scheduling, supportive leadership, and a high level of autonomy — all while serving your local community with integrity. **Key Responsibilities:** * Deliver in-home physical therapy services under the supervision of a licensed Physical Therapist * Willingness to travel to Truth or Consequences one to two days a week with mileage compensation * Carry out treatment plans to help patients regain mobility, function, and independence * Communicate progress and changes in patient condition to the supervising therapist * Educate patients and caregivers on exercises and safety practices * Document visits and progress notes accurately and on time * Participate in care team meetings and interdisciplinary planning * Uphold infection control and safety standards during all home visits * Maintain professional, compassionate rapport with patients and families Requirements: **What We’re Looking For:** * Current licensure as a Physical Therapist Assistant in New Mexico * Previous experience in home health preferred, but not required * Strong interpersonal and communication skills * Confidence working independently in the field * Adaptability to changing schedules and home environments * Commitment to quality, safety, and patient-centered care * Reliable transportation and valid driver's license * Occasional Travel to T or C preferred **Why Join Us:** Sierra Home Health is part of a nonprofit healthcare organization managed by CareM.
