Physical Therapist

**Job Summary** We are seeking a dedicated and compassionate Physical Therapist to join our dynamic healthcare team. In this vital role, you will assess, develop, and implement personalized treatment plans to improve patients’ mobility, reduce pain, and enhance overall quality of life. You will work closely with a multidisciplinary team to deliver exceptional patient care across various settings, including outpatient clinics, inpatient hospitals, home health environments, and community care programs. Your expertise in physiotherapy and strong communication skills will ensure patients receive the highest standard of rehabilitation services tailored to their unique needs. **Responsibilities** * Conduct comprehensive patient assessments to determine physical therapy needs, including evaluating mobility, strength, and functional limitations * Develop and implement individualized treatment plans utilizing evidence\-based practices such as manual therapy, trigger point therapy, and therapeutic exercises * Collaborate effectively with healthcare professionals including physicians, nurses, occupational therapists, and social workers to coordinate patient care * Manage clinical documentation standards by accurately recording treatment progress, discharge planning details, and patient responses in electronic health records (EHR) systems * Provide patient education on injury prevention, home exercise programs, and community resources to promote ongoing recovery and wellness * Assist with discharge planning by evaluating patient progress and recommending appropriate follow\-up care or referrals * Work with diverse patient populations including geriatric patients, pediatrics, neurology cases, orthopedics, and those requiring inpatient or outpatient rehabilitation **Skills** * Strong knowledge of physiology, anatomy, medical terminology, and physiotherapy techniques relevant to diverse clinical settings * Proven experience in outpatient clinic environments, inpatient hospital units, home health services or outpatient rehabilitation centers * Expertise in treatment plan development for various conditions such as musculoskeletal injuries, neurological disorders, geriatrics and pediatrics * Proficiency in electronic health records (EHR) management and electronic medical records (EMR) systems for accurate documentation * Ability to perform discharge planning effectively while adhering to clinical documentation standards * Excellent client communication skills with a focus on empathetic patient care and education * Familiarity with outpatient experience including outpatient rehabilitation center operations and outpatient clinic workflows Join us in making a meaningful difference by helping patients regain independence through expert physical therapy!
